OUT OF NETWORK PROVIDER
Services may be eligible for reimbursement as an out-of-network provider for all other insurance plans. This means that services may be covered in part by your health insurance or employee benefit plan. Full payment will be due at time of services, and I will provide you with an insurance statement that you can submit to your insurance company for out-of-network reimbursement.
If you plan to use insurance, please contact your insurance provider directly by calling the customer service number on the back of your insurance card and ask the following questions:
• Does your plan cover behavior health?
​​• What is the coverage/ copay for an in network provider and out of network provider per visit?
• Is there a limit on the number of behavior health visits?
• Do my behavior health services go toward my deductible?• Is a preauthorization needed for behavioral health services?

Consultations- Free
I am aware seeking mental health support can be a difficult process. I want to ensure I'm meeting your needs and expectations for treatment. By allowing a 15–20-minute call to discuss your needs, you can gain a greater understanding of this practice and allow you to make an informed decision if you choose to move forward with therapeutic supports.

Completion of Client Portal and Consent for Services
Once you decided to move forward with support, a client portal link will be provided to complete necessary paperwork to start services. An initial visit and assessment appointment will be schedule.

Initial Visit and Assessment
This visit will allow me to complete a clinical assessment to determine the areas of concerns, start goal setting, and provide a clinical diagnosis of current mental health symptoms. The initial sessions provide an opportunity for you to determine whether you feel comfortable and safe. I will also be assessing whether our practice is best to meet your individual needs.

On Going Sessions
During ongoing services, therapeutic interventions will be provided to address your goals and needs. The frequency of sessions will be determined to fit your individual needs and schedule.
